The System Navigator partners with the Family and Children First Council of Cuyahoga County and the Service Coordination Team (SCT) to serve multi-need, multisystem youth to prevent them from falling through the cracks due to intake, eligibility, or funding barriers.

In Cuyahoga County, Service Coordination is a family-driven process designed to bring services and supports to children and families in a manner that included family participation at every level.

If a youth is working with the SCT, the System Navigator can assist the school district in understanding the process, clarifying the role of the district, or other partners, and linkage to the team.
